    When filing a request for investigation of sexual harassment, the following information should be provided:<\/h1><\/div><\/div><\/div>
      \n \t
    1. The name of the applicant or the whistleblower, ID document number, the department and title where the person is employed or enrolled, residential address, contact phone number, and the date of the investigation request.<\/span><\/li>\n \t
    2. If the applicant is requesting an investigation, the birthdate of the victim should be specified.<\/span><\/li>\n \t
    3. If the applicant has appointed a representative to file the investigation request, an letter of authorization must be attached, which should include the representative's name, ID document and number, residential address, and contact phone number.<\/span><\/li>\n \t
